Second Edition represents a complete revision of the first edition of this manual, published in 1989. Although there are approximately the same number of chapters, the amount of information has been expanded considerably, arranged in an easily accessible outline format. Furthermore, while the number of radiologic and other photographs has been reduced, the number of tables, charts, and drawings has increased substantially.
